Description 
In this panel discussion, clinicians will learn from five boots on the ground clinicians who are effectively implementing a spectrum of nutritional interventions in a variety of patient populations. Find out how these clinicians are leveraging behavior change techniques to empower dietary change in the real world. Learn how providers are addressing and overcoming common barriers for implementing tailored nutrition interventions in a variety of patient populations including the social determinants of health, family dynamics, mental and emotional status, cultural implications, reimbursement issues and more. Each panelist will also share their favorite educational tools and resources that they’ve found to work in their practice.

A WFPB dietary approach supports women’s health across the lifespan. Description 
This session will highlight the new, and first, multidisciplinary Expert Consensus Statement (ECS) developed by the American College of Lifestyle Medicine (ACLM) on Dietary Interventions to Treat Type 2 Diabetes (T2D) in Adults with a Goal of Remission. The objective of the ECS is help clinicians achieve remission of type 2 diabetes (T2D) in adults using diet as a primary intervention, by providing evidence-informed statements agreed upon by experts from the American College of Lifestyle Medicine, American Association of Clinical Endocrinology (AACE), American Academy of Family Physicians (AAFP), American College of Cardiology (ACC), American Heart Association (AHA), Academy of Nutrition and Dietetics (AND), and the Endocrine Society (ES). Using a valid and trustworthy process, which included iterative Delphi surveys, the panel reached consensus on 68 statements related to definitions, diet as a primary intervention for remission of T2D, dietary specifics, adjuvant interventions, monitoring therapy, weight loss, and payment and policy. Clinicians can use these statements to improve quality of care, inform policy and protocols and to identify areas of uncertainty.

  • Define remission for T2D in adults based on minimum time and glycemic measures

Description

Within the Health Systems Council, ACLM is working closely with nearly thirty health systems to better understand the strategies and implementation models that they are using to integrate lifestyle medicine into their clinical care models, both for patients and also for employees. These approaches are diverse and range from specialized primary care practices to centralized lifestyle medicine clinics referred into from multiple business lines. What they often share in common is a highly specialized lifestyle medicine trained workforce leading the efforts. In this session we hear directly from four of these visionary leaders, representing St. Luke’s Health, Adventist Health, UPMC and Vidant Health.

  • Review replicable multifaceted approaches that leading health systems are utilizing to effectively implement LM solutions. 
  • Explore strategies to create a culture focused on health and well-being in a large healthcare system.

Behavior change is the foundation for effective lifestyle prescriptions, yet the path to lasting lifestyle change is complex, individualized and influenced by a multitude of factors.
