Become a Member

We welcome you to apply for professional membership in the American College of Lifestyle Medicine. Be part of this exciting, new medical specialty.

We process new member applications on a monthly basis. To be considered complete an application requires:

•  A completed application form (can be done online below)
  
•  A CV or resume (sent separately)
•  Copy of license or certification (if required, see explanation below)
•  Application payment which includes the annual dues and registration fee.

Practitioners who check "yes" after the question "Are you accepting patients?" are required to provide a copy of their current license or certification.

A one-time $35 registration fee applies to all new members, and is included in the amounts listed below.

Membership is renewed annually on January 1, but we do pro-rate renewals for members who join after April 1.

Once accepted for membership, "new" members become current members.
