What is LEADR? 

  • A 24-session lifestyle intervention designed to treat prediabetes and type 2 diabetes 
  • Emphasis on lifestyle change and providing practical tools for eating a delicious and nutritious whole-food, plant-predominant diet for both the patient and their families 
  • Content is structured around setting goals that are specific, measurable, achievable, relevant, and time-bound (SMART goals)

Community Feedback on Healthcare Approaches to Supporting Lifestyle Change and the LEADR Curriculum

Help the ACLM Research Department ensure that the LEADR curriculum reflects the needs of patients by recruiting patients with diabetes to give interviews on their experiences.    

All you need to do is be willing to share our recruitment announcements, available in English or Spanish, with your patients via email, social media postings, and/or by posting or sharing recruitment flyers at your office.  

This study utilizes one-on-one interviews to ask patients about their experience with diabetes and willingness to adopt lifestyle change, including barriers to change.  The study is conducted remotely using Zoom. 

We hope that this process will ensure that the curriculum and materials are both acceptable and appropriately tailored to patient’s needs. 

Patients who participate in the study will receive a small stipend for their time.

How can LEADR help patients? 

Empower patients to take control of their health through long-term behavior change for the patient and their family 

Specifically, patients will learn to: 

  • Eat for diabetes remission without complicated calorie counting
  • Work towards a healthy weight without feeling hungry
  • Improve energy levels and quality of life

For some patients, diabetes remission may be possible! 

How can LEADR support providers? 

The program was created with a focus on sustainable implementation for practitioners.  Sessions are designed as shared medical appointments (SMAs) that can be billed through CMS or other insurance. Pilot-testing with a focus on testing reimbursement is underway. If you would like to run a pilot session of LEADR in your practice, please click below. 

LEADR is flexible!  While the original curriculum is divided into 24 sessions, it offers flexibility for practitioners to combine the content into fewer sessions if needed. The format can be delivered virtually, in-person, or hybrid.
