Accelerating healthcare transformation through systems change

Healthcare in the U.S. has reached a tipping point where the rising burden of chronic presents not just a challenge but an opportunity to re-imagine what healthcare can and should be. ACLM’s Innovators Council exists to accelerate this necessary paradigm shift through cross-sector collaboration.

Padmaja Patel, MD, FACLM

“A true paradigm shift requires more than innovation at the edges; it demands transformation at the core. To meaningfully change the healthcare landscape, the way we deliver, incentivize, and measure care must be fundamentally aligned with the goal of health restoration, not just disease management.”

Leading the shift from sick care to true health care

The Innovators Council brings together leading risk-bearing entities – including health systems, health plans, ACOs, and self-funded employers—who are united in transforming care delivery through the systematic integration of lifestyle medicine.

This by-invitation working council is designed for mission-aligned organizations committed to rapid, scalable, and measurable impact. Together, we are not just thinking big, we are working strategically to advance the Quintuple Aim.
