Enrollment: Lifestyle Medicine Residency Curriculum

Please follow the instructions below to successfully complete enrollment into the Lifestyle Medicine Residency Curriculum (LMRC).

1. Click ‘Enroll Now‘
2. Create a new account or sign in to your existing ACLM account.
3. Fill out the entirety of the LMRC Enrollment Form, making sure to indicate your role (resident, faculty, or fellow). Choose your residency site and program in the ‘Program Submission’ tab.’
4. Click ‘Finish’ at the end of the form.
5. Allow 2-3 business days for processing. You will receive a confirmation email when your enrollment has been approved and successfully processed.

• Please note there may be multiple programs at each site implementing the LMRC. When asked to indicate your program on the program Submission’ tab, make sure to select the correct program offering at your site.
