Lifestyle Empowerment Approach for Diabetes Remission

Lifestyle Empowerment Approach for Diabetes Remission (LEADR) is a lifestyle medicine program delivered through a shared medical appointment (SMA) model that helps clinicians guide patients toward type 2 diabetes remission.

The program provides a practical, reimbursable framework for group medical visits, along with step-by-step tools clinicians can use to implement lifestyle medicine in real-world practice. Through LEADR, patients learn how to eat for diabetes remission without calorie counting, achieve a healthy weight without hunger, and improve their energy, metabolic health, and overall quality of life through sustainable lifestyle change.

Materials

Designed for flexible delivery over 12 weeks—in person, online, or in a hybrid format—the framework includes a comprehensive Facilitator Script and Participant Guides that help ensure consistent implementation and strong patient outcomes.

This program will help your participants refine their dietary and lifestyle habits to improve their blood glucose and pursue a path toward remission.

Participants will:

  • Learn how to improve their dietary habits by incorporating fiber-rich whole foods (legumes, whole grains, vegetables, starchy roots and grains, and whole fruit
  • Set attainable goals each session to reach their set vision toward T2D remission objectives
  • Encounter inspiration, education, and guidance towards optimal lifestyle habits. LEADR is couched in a flexible format to meet your delivery parameters

The LEADR design is scalable to your group, and flexible to deliver optional sections if time allows. Some can be assigned to participants between meetings, and some are optional. LEADR contains 12 Teaching Kitchen segments that can be used in a variety of ways:

  • Conduct live cooking demonstrations in the meetings
  • Prepare food ahead, bring in samples, and discuss while tasting
  • Use the content simply as education and talk through the cooking in the meetings

Sessions can be held in-person, online, or a combination of the two.
