ACLM’s Innovators Council brings together leading risk bearing entities, including health systems, health plans, ACOs and self-funded employers, who are united in the pursuit of healthcare transformation achieved through the Quintuple Aim.
COLLECTIVE IMPACT
Innovators Council members work together in a collective impact model to unpack the complexity of systematic adoption and implementation of lifestyle medicine at-scale. Through open knowledge exchange, Council members unearth shared challenges and develop novel pathways to effect accelerated change.

Eligibility:
To be considered for the Innovators Council, organizations must fall into one of four categories: health system, health plan, ACO, self-funded employer.
